Fatal mistake?

Was it medical negligence or fatal mistake?

It was supposed to be a simple procedure but a routine operation cost a Victorian father of four his wife.

Bass Coast Regional Health issued ACA with the following statement:

"Whilst Bass Coast Regional Health is unable to make any specific comment on Mrs Richards case given a Coronial investigation is underway, patient safety at bass Coast Regional Health continues to be of the utmost importance.

Bass Coast Regional Health is co-operating fully with the Coroner's investigation and, in the meantime, has facilitated open disclosure meetings with Mr. Richards to express its regrets regarding the passing of Mrs Richards and to keep him informed of the investigations that are being undertaken."
